When and how to report a claim or ask questions
Please notify us as soon as you become aware of facts which cause you to assume that a loss or incident which may result in a claim under your policy has been incurred or will be incurred. You do not need to be aware of all the details or the amount of loss. Please file a loss notice via email to the extent you have information available as shown below. If you do not have access to email, please fax, mail or call us with the details regarding the claim.
Loss Notice Information Required from the Insured
The loss notice should include the following:
- Name and address of the insured, contact details, policy number and effective dates
- Date of Loss
- Location of Loss
- Written narrative of cause of loss
- All pertinent letters or documentation necessary to properly evaluate the claim
Please provide duplicate copies of all claim notices and correspondence sent directly to Berkley Crime to your professional agent or broker.
